Hiring Salary Range: Step 1 $3,706.35/mo. – Step 3 $4,169.33/mo. DOE

JOB SUMMARY:  Provides outreach, engagement, and intensive case management services to individuals referred by law enforcement, community-based organizations, emergency medical services, and other individuals and organizations who might come in contact with an individual who could benefit from RNP program services.  The Case Manager performs outreach, long-term engagement and supportive services for participants through intensive case management activities and collaboration with Managed Care Organizations, local partners, service providers, housing providers and other community organizations.  This position is non-exempt and subject to a collective bargaining agreement.

ESSENTIAL FUNCTIONS: 

• Must work effectively with the client, public, community partners and other employees.
• Maintain a participant caseload and referrals, within the uniform program standards.
• Engage participants in the community, to establish a working relationship and offer services.
• Provide assessment of Individuals’ needs to determine stressors and targeted intervention points.
• Assist participants in gaining access to a variety of programs (e.g., Medicaid, SSI, housing, et); assist participants in navigating barriers to access.
• Assist with developing a person-centered, individualized service plan based upon individual strengths.
• Develop and maintain a working relationship with the emergency response, law enforcement,  fire departments, social and health provider network, and other community    organizations which may support participants.
• Provide structured Intensive Case Management services consistent with program policies.
• Provide advocacy and support for participants within the criminal justice system including court appearances and written communication.
• Provide crisis response support, within program standards, to include after-hours.
• Attend stakeholder work groups and committees to represent the experiences of program participants.
• Complete all required trainings within identified time frames.
• Complete all required reporting within identified time frames.
• Be familiar and follow all state and federal laws relevant to position and scope of responsibilities.
• Be familiar and follow applicable departments and all County Personnel Policies and Procedures.
• Be familiar with grant and contractual requirement relevant to position.
• Perform other duties and assignments as requested by the RNP Supervisor or Clinical Director

KNOWLEDGE AND ABILITIES: 

• Possess knowledge and skills of case management approaches.Knowledgeable of harm reduction strategies, motivational interviewing and relevant evidence-based practices.
• Knowledgeable of common mental health and substance use diagnosis.
• Familiar with outreach engagement approaches, service referral and available community resources, recovery supports and organizations for those with substance use and co-occurring disorders; the regional system of care including mental health agencies and substance abuse and recovery services; Community resources, housing providers; federal and state entitled benefits, and other programs individuals may qualify.
• Able to work both independently and as a member of a team.
• Ability to maintain a professional demeanor and effectively communicate in high stress situations.
• Able to effectively support culturally diverse, and socioeconomically disadvantaged, individuals with disabilities, and populations disproportionately impacted by the criminal legal system
• Able exercise tact and discretion in obtaining cooperation of others.
• Skills necessary to provide advocacy and support for participants within the criminal justice system including court appearances and written communication. Ability to effectively communicate and problem solve under pressure in high stress situations.
• Able to exercise discretion and independent judgment in all areas of job performance, including adherence to appropriate professional boundaries and strict confidentiality practices;  relevant Federal and State laws, rules and regulations, to include HIPAA and 42 CFR part 2.
• Familiar with outreach engagement approaches, service referral and available community resources, recovery supports and organizations for those with substance use and co-occurring disorders; the regional system of care including mental health agencies and substance abuse and recovery services;  Community resources, housing providers; federal and state entitled benefits, and other programs individuals may qualify.
• Able to manage a caseload of individuals at varying stages of recovery.
• Possess skills necessary to provide advocacy and support for participants within the criminal justice system including court appearances and written communication. Must have the ability to effectively communicate and problem solve under pressure in high stress situations.
• Possess effective time management skills in coordinating daily activities, appointments and completing necessary paperwork.
• Operate standard office equipment, (e.g., computer, cell phone, agency owned vehicles, et.).

EDUCATION AND EXPERIENCE: 

• High school diploma or equivalent. Bachelor’s degree in social work, psychology or related human services field from an accredited institution – Preferred.
• Minimum three years professional experience working in community mental health, Preferred.
• Preferred lived experience, defined as a current or past recipient of behavioral health services and/or with current or past involvement in the criminal legal system.
• Experience working with individuals with complex life issues and/or behavioral health conditions preferred. Experience providing street-based outreach and engagement highly desired.

LICENSES AND OTHER REQUIREMENTS:

• A Washington state Department of Health Agency Affiliated Counselor certification -Preferred
• A valid National Provider Identification Number (NPI) – Preferred.
• Valid driver’s license required.
• Must successfully pass a background and a driving record check.
• Is not a State or Federal (OIG) and System for Award Management (SAM) excluded provider, in any capacity for the Medicare, Medicaid, and all Federal health Care programs.
• Must be in active recovery, and able to provide appropriate case management and support services to program participants without impaired ability to remain in active recovery.
